If you need a rapid antigen test (15 minute test) or a PCR test sent to our lab, we can help!  We offer multiple solutions both onsite and Telemedicine visits including:

Please call our office before you make payment to determine what you need and how we can be an assistance. 

Once you have spoken to us and decided what is your plan, you can: 

• Make the payment
• Call or text 815-600-3053 and let us know you are ready for your televisit if you want to be seen during business hours 8-5 or set up an appointment for another time.
